Customize survey templates

Platform: ResiOnboarding · For: Executive Director, Regional Directors, Owners

Default surveys are ready to use. You can clone one to change it, or build your own. Only Executive Directors, Regional Directors and Owners can do this.

Clone a default

1. Choose Surveys and open the Templates tab.
2. Select Clone on a default survey.
3. On your copy, select the edit icon.

Build your own

1. Select Create Survey.
2. Enter a Survey Name, choose a Checkpoint, and add a Description.
3. For each question, enter the text, a Section and a Type: Scale (1-5), Single Choice, Multi Choice or Open Text.
4. Select Add Question for more.
5. Select Create Survey.

Add an alert trigger

1. On a question, turn on Alert Trigger.
2. Set the rule. For a scale, you might flag an answer of 2 or lower.

Open Text questions cannot have triggers.

Good to know

• Sections become the pages a resident moves through.
• Use the arrows on a question to reorder it.
• Default templates cannot be edited directly. Clone them first.

If something goes wrong

"Cannot delete" means the template has been assigned to residents. Those assignments must be removed first.
