A few friendly questions help the team get to know a resident as a person.
If you are a family member
- Open the link from the community team. You don't need an account.
- Answer as many or as few questions as you like. Select Next, or Skip.
- On the last step, review your answers.
- Enter your name and your relationship.
- Select Share this story.
If you are the resident
- On Home, find Share your life story and select Start the interview.
- Answer the questions you want to.
- Select Save my story.
What we ask
The questions cover where you grew up, work, hobbies, important people, traditions, favorite foods, a favorite story and a perfect day.
Good to know
- You can leave and come back on the same device. Your answers are saved there until you send them.
- The community team reads your answers and adds them to the resident's life profile.
- Please don't share medical details here. Talk to the community directly about care.
If something goes wrong
"This link is no longer active." Links expire for privacy. Ask the community team for a fresh link.
"We couldn't open this page just now." Check your internet connection and select Try again.
For questions about care or an apartment, call the community. In an emergency, call 911 or use the resident's emergency call system.
